In-Home Assessment/Coordination

Becoming a SENIOR CHOICES client is an easy but thorough process that assures you will receive services that will help you to remain in your own home. This process consist of three steps:

HOME CARE ENROLLMENT PROCESS

Formal enrollment into the Senior Choices home care program begins with our Intake and Referral Specialist. Usually this person will ask some general questions and gather some basic information about prospective clients. At this point,  a Care Consultant will be assigned and make contact with the client to arrange for the in-home assessment. The Care Consultant will bring an enrollment packet with them or in some cases the packet will be mailed to the client first.

COST CONSIDERATION

Eligibility for SENIOR CHOICES services is not based on income. Those whose income and personal assets prevent them from affording services privately, are eligible for assistance through our Senior Service levy at a reduced cost, or if determined to be eligible, at no cost at all. SENIOR CHOICES clients receive one convenient monthly statement detailing all services and related costs.

Currently about 55% of individuals enrolled in SENIOR CHOICES pay nothing for their services.

CARE COORDINATION

As a SENIOR CHOICES client, you are provided with a personal Care Consultant, free of charge, who acts as your advocate during the entire time you receive assistance from the Council. Your Care Consultant is in regular contact with more than 24 providers of in home and community-based care who are under contract with the Council for Older Adults.

Our Care Consultants first priority is to help you address your individual needs. With your needs in mind, personal Care Consultants make all arrangements for in-home services, monitor that assistance to ensure quality and adjust services as your needs change. This "one-stop" approach offers the best in coordinated assistance, suited to individual needs.

Each of the Council’s Care Consultants is a professionally trained and licensed social worker with specific interest in the field of geriatrics. Additionally, all social workers on the staff of the Council have earned their Masters of Social Work degree or are supervised by a Master’s level Social Worker. We are very proud of our ability to offer you this level of high quality and experienced assistance when you, a family member or friend need help.
