Council for Older Adults Accepting Applications for Senior Service Providers

The Council for Older Adults is seeking to contract with qualified providers of in-home and community-based services for 2008 and 2009 under its Senior Choices program.  Not-for-profit, public and proprietary firms may request application materials, Conditions of Participation and Service Specifications for the following services: adult day care, homemaker and homemaker escort services, mental health counseling, nursing services, personal care, respite, institutional respite, emergency response, medical transportation, durable medical equipment and supplies, minor home repair/modification and chore services. To be eligible for consideration for approval as a service provider beginning January 1, 2008 your application must be submitted to the Council no later than July 30, 2007.
